- IDT (Interdisciplinary Team)
- The group of professionals — nurses, therapists, social workers, dietitians, and others — who create and update a resident's care plan.
- SNF (Skilled Nursing Facility)
- A facility that provides 24-hour nursing care and rehabilitation, often after a hospital stay.
- ALF (Assisted Living Facility)
- A residential setting that offers help with daily activities like meals, medication reminders, and personal care.
- Discharge planning
- The process of preparing a patient to leave one care setting and move safely to the next, whether home or another facility.
- Care plan meeting
- A regular meeting where the care team reviews the resident's goals, progress, and any needed changes to treatment or services.

- Federal and state protections that guarantee residents dignity, privacy, choice, and participation in their own care decisions.
- Private group home
- A residential care setting that supports a small group of residents, ranging from those who live mostly independently to those who need more daily assistance. In Virginia, these homes are often regulated by the Department of Social Services.
- Home Health
- Skilled care provided by licensed professionals in your own home — such as nursing, physical therapy, or wound care. For Medicare recipients, it is typically time-limited and ordered by a physician.
